Job Summary

• Position: Urology
• Start Date: October 13, 2025
• End Date: January 12, 2026
• Duration of Assignment: 91 days
• Care Setting: Clinic and call coverage
• Shift Details: Clinic 7 am - 5 pm, Night call 5 pm - 7 am
• Number of Patients per Day: 2-5
• Weekend Coverage: Occasional weekends needed

• Licensure: Indiana license or IMLC
• Board Certified or Board Eligible within 5 years of residency
• Certifications: ACLS, CDS, DEA Registration
• Robotics Trained: Highly Preferred
• Vaccination: Required
• Credentialing Timeline: 90-120 days for payer enrollment, 45-60 days for hospital privileges
