Responsibilities

• Provide direct resident care including nursing/clinical care, personal, nutritional and restorative care in the skilled nursing operations.
• Demonstrate the Sunrise Mission: to champion quality of life for all seniors, in accordance with standards, regulations and Sunrise policies.
• Deliver resident care duties per Individualized Service Plans (ISP).
• Report all incidents immediately to the supervisor or appropriate team member; report suspected abuse per regulations.
• Participate in care planning for designated residents; perform individualized personal care and activities of daily living (ADL); document ADL in flow sheets.
• Partner with licensed nurses to plan and implement restorative therapy programs and activities for residents.
• Obtain and report vital signs, urine output and meal intake per the care plan to the licensed nurse.
• Observe and report changes in condition requiring licensed nurse intervention.
• Maintain assistive devices and equipment; keep equipment clean and available.
• Maintain Restorative Records for residents.
• Maintain knowledge of applicable resident care regulations.

Quality Indicators

• Aim for excellent quality care and service delivery as measured by QAPI.
• Demonstrate behavior interventions and management competencies; knowledge of ADL, skin care, dining assistance, bowel/bladder elimination, falls and fall prevention, and Resident-Centered Care model.

Guest/Resident, Family and Visitor Relations

• Respond to inquiries and concerns in a timely, professional manner using the Problem Resolution process.
• Invite, encourage and assist residents to meals; assist during transfers or discharges; escort to transportation.
• Greet new residents and orient them to their room, call lights, and community layout; provide activity calendar.
• Inventory and label belongings at admission and ongoing thereafter.

Risk Management and General Safety

• Ensure community compliance with health and safety regulations; adhere to safety rules and PPE, fire extinguishers, SDS, and lockout/tagout procedures.
• Practice safety procedures, including handling of bloodborne pathogens and hazardous materials; report exposures immediately.
• Ensure safe storage of oxygen tanks and monitor prescribed oxygen levels.

Training and Team Contribution

• Participate as a team member; contribute to engagement programs and attend regular meetings.
• Adhere to Sunrise Principles of Service and Team Member Credo.
• Complete required trainings and in-services; perform other duties as assigned.

Experience and Qualifications

• Valid Certified Nursing Assistant license/certificate per regulations
• High School Diploma or GED required
• Previous CNA experience preferred
• Desire to serve and care for seniors
• Ability to handle multiple priorities; strong communication and documentation skills
• Strong organizational and time management skills; good judgment and problem-solving abilities
• Ability to work weekends, evenings and flexible hours
• Ability to stay calm in difficult situations and be considerate
