Job Description Job Title: Hospice RN, Home Health RN Profession: Nursing Specialty: Hospice RN, Home Health RN Duration: 13 weeks Shift: M-F Hours per Shift: 8 am - 4 pm with some weekend and on-call required Experience: Minimum of 2 years License: Valid nursing license Certifications: CPR certification

Must-Have: - Strong knowledge of OASIS documentation requirements - High-Tech skills including IV use and maintenance, wound VAC, complex wound care - Ability to assess patient and caregiver needs - Experience in developing and implementing care plans - Excellent documentation and communication skills

Description: Effectively fills out and completes OASIS documentation in accordance with agency policies and procedures. Demonstrates knowledge of, and keeps up-to-date on changes to OASIS documentation requirements as needed. Ensures documentation is completed in accordance with agency practice and timelines. Utilizes the nursing process to assess patient and caregiver needs in order to provide quality care. Develops and communicates a realistic plan of care for each patient. Ensures that patient and caregivers are included in the care plan process. Serves as Case Manager for assigned patients and actively manages caseloads and visit times. Implements medical plan of care, nursing interventions, and procedures as necessary. Stays up-to-date on a variety of High-Tech skills and maintains infection control standards. Works collaboratively with LPNs to ensure that the plan of care is followed. Consistently reviews documentation and acts on recommendations. Evaluates a variety of aspects of the implemented plan of care, including problems identified and patient responses. Maintains up-to-date and accurate documentation of nursing care provided. Follows agency protocol with relevant processes. Performs patient and caregiver education to ensure informed patient decisions can be made. Identifies interdisciplinary needs of patients and documents accordingly. Communicates documented information to other disciplines in a timely manner. Accurately completes a variety of agency forms in a timely manner. Carries out visits efficiently, ensuring supervisory visits are not combined with routine visits. Assesses patient needs for Home Care Assistant services and coordinates plans of care. Transmits patient incident reports to Quality Improvement Specialist in a timely manner. Coordinates safe patient discharges and transfers, processing all appropriate paperwork. Coordinates patient care with physicians, providers, and team members to ensure effective support of the plan of care. Participates in monthly peer review audits and provides constructive feedback. Proactively schedules and facilitates case conferences with team members. Advocates for patient care issues to ensure overall quality and sensitivity to patient and family needs.
