The

RN Nurse Navigator - Breast Oncology

serves as a clinical resource and patient advocate, guiding individuals and families through the breast cancer care continuum. This role utilizes oncology-specific nursing expertise to provide education, counseling, care coordination, and ongoing support. The Nurse Navigator facilitates timely access to diagnostic, treatment, and supportive services while ensuring patients receive age-appropriate, culturally sensitive, and patient-centered care.

Essential Functions:

Patient Navigation & Support Serve as the primary point of contact for patients and families during the diagnostic and treatment process. Provide education about disease, treatment options, side effects, and available resources. Assist patients and caregivers with decision-making by offering guidance, information, and emotional support. Coordinate appointments, referrals, and treatment planning across the care continuum.

Clinical Care & Education

Perform triage and telephone advice independently within scope of practice. Conduct nursing assessments and interventions during in-person or telehealth encounters. Deliver age-specific, culturally appropriate education and health promotion. Support patient advocacy, promote shared decision-making, and ensure continuity of care.

Care Coordination & Case Management

Expedite access to diagnostic and treatment services. Collaborate with physicians, advanced practice providers, and multidisciplinary teams to align care with established standards and protocols. Track and document outcomes and patient metrics in electronic medical records (Epic EMR preferred). Facilitate referrals to support services, community resources, and survivorship programs.

Professional Practice & Compliance

Adhere to nursing standards of practice, HIPAA, HR, safety, and compliance requirements. Participate in quality improvement initiatives, process redesign, and performance measurement. Contribute to the overall patient experience and satisfaction. May organize or lead support groups and assist with translation services as needed.

Understanding of medical terminology, medications, dosages, and side effect management.

Strong critical reasoning, triage, and clinical judgment skills for complex or urgent patient situations. Demonstrated ability to teach, educate, and counsel patients and caregivers. Skilled in patient advocacy, care coordination, and multidisciplinary collaboration. Proficiency with electronic health record (EHR) systems and medical practice management software. Strong organizational, communication, and problem-solving skills. Ability to provide compassionate, culturally sensitive care that respects patient values and preferences. Minimum Requirements:

Education & Certification

Bachelor of Science in Nursing (BSN) or equivalent required. Current

California Registered Nurse (RN) license

required. Basic Life Support (BLS)

certification - American Heart Association required. Minimum of

two (2) years of progressively responsible RN experience

in an ambulatory or outpatient setting. Oncology and/or infusion therapy experience

strongly preferred. Epic EMR experience preferred. Certified Oncology Nurse-Required Certified Nurse Navigator-Preferred Experience

Ability to work in a fast-paced clinical setting. May involve sitting, standing, walking, and occasional lifting up to 25 lbs. Requires flexibility to work varied hours as needed for patient care and navigation support.
