Infection Preventionist Duties
Develop, implement, and monitor infection control policies and procedures for the higher levels of living (SNF, AL, Memory Care) Conduct ongoing surveillance of healthcare-associated infections (HAIs). Lead outbreak investigations and coordinate response efforts. Educate staff on infection prevention practices, including hand hygiene, PPE use, and isolation protocols. Collaborate with local and state health departments for reporting and compliance (Local county health department, IDPH, NHSN) Oversee fit testing for all team members according to OSHA Emergency Temporary Standards Oversee Infection Control compliance related to Employee Health (TB screening, immunizations) Perform environmental rounds and audits to assess infection control practices. Active member of the QAPI Committee Nurse Supervisor Duties
Supervise nursing staff across all shifts, ensuring appropriate staffing and skill mix in collaboration with Scheduler. Provide clinical guidance and support to nursing personnel. Monitor care delivery for compliance with standards, policies, and regulations. Participate in performance evaluations, coaching, and disciplinary actions. Assist with onboarding, training, and competency assessments of nursing staff. Respond to emergencies and escalate clinical concerns appropriately Be part of the on-call rotation schedule for higher levels of living (SNF, AL, Memory Care) Integrated Responsibilities
Ensure infection control protocols are embedded in daily nursing operations. Lead interdisciplinary meetings focused on infection risk and clinical quality. Support staff in balancing resident care needs with infection prevention strategies. Maintain documentation and reporting systems for both nursing supervision and infection control . Comply with applicable federal, state and local laws, rules, and regulations; maintains knowledge of and follows all Plymouth Place policies and procedures. Maintain regular and punctual attendance at work and meetings. Attends in-service and education programs. OTHER DUTIES:

JOB REQUIREMENTS:
Registered Nurse (RN) with active state licensure. Minimum 3 years of clinical nursing experience; supervisory experience preferred. Required Certification in Infection Control - Long Term Care (CDC). Basic Life Support (CPR) certification. KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:
• Knowledge of CMS infection prevention regulations, CDC guidelines, and state survey requirements specific to long-term care.
• Strong understanding of microbiology, disease transmission, and outbreak management in skilled nursing, assisted living, and memory care.
• Skill in conducting surveillance, analyzing infection trends, and reporting data.
• Effective communication and teaching skills to train and coach staff across all departments.
• Ability to lead the Infection Prevention and Control Program (IPCP), respond to outbreaks, and collaborate with interdisciplinary teams.
• Ability to adapt infection control practices to meet the unique needs of older adults and residents with cognitive impairments.
Understanding of Point Click Care or similar systems. Ability to cover the weekends, evenings or holidays as needed. Excellent communication and interpersonal skills. Ability to work independently and multitask. Proficiency with Microsoft Office Suite or related software. Strong organizational skills and attention to detail. Ability to read, write, and speak English sufficiently to perform job duties and interact with residents, families, and employees.
ENVIRONMENTAL AND PHYSICAL ACTIVITY REQUIREMENTS
: The physical activities of this position constantly require effective talking, hearing, sitting, walking, and standing functions. The position also often requires exerting up to 50 pounds of force occasionally and/or negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Must be able to perform activities with close visual acuity, such as reading a computer screen and reading print material. The functions of this position are conducted in a temperature-controlled environment.
OSHA Exposure Category:
Category I - Position includes tasks that involve exposure to blood, body fluids, tissues; would be required to perform Category I tasks.
INTELLECTUAL/EMOTIONAL REQUIREMENTS:
The occupant of this position must be able to perform the responsibilities of the position well under pressure. The position requires adaptability to performing a variety of duties, often changing from one task to another without loss of efficiency or composure. This position requires the ability to deal with and find resolutions to problems and maintain a productive working relationship with employees and may be interrupted frequently to meet the needs and requests of others. It also requires adaptability to fluctuate between priorities daily while being responsive to needs as they arise.
