Case Manager
for our Harrisburg, PA senior living office. This office provides best in class care to clients throughout Harrisburg, PA and the surrounding areas.

Responsibilities for a Case Manager:
Lead, inspire and motivate others to provide exceptional care.
Visit our Senior clients and conduct assessments in Harrisburg, PA and other surrounding areas.
Develop the care plan and oversee its execution.
Supervise home health aide (HHA) staff in the field.
Facilitate new hire orientation.
Conduct competencies on new and tenured staff
Requirement(s) for Role:
Must have 2 years of home health experience
