Job Title:
Community Relations Liaison
Position Summary: The Community Relations Liaison (CRL) builds and maintains positive relationships with professional referral sources and clients to foster a strong network and promote the agency's home care services. The CRL acts as the main contact for referral sources, representing the agency at networking events and community meetings. This role involves planning and managing outreach activities to position the agency as a trusted "go-to" home care provider, focusing on educating the community about the agency's values, services, and mission.
Key Responsibilities:
Spend 80% of work time visiting referral sources, with a goal of 30 visits per week
Attend at least one professional networking meeting and deliver one presentation weekly
Generate new client starts through professional referrals, aiming for a 50% closing rate
Act as the face of the agency in the territory and cultivate relationships with referral sources
Target clients with chronic conditions such as Alzheimer's, Dementia, Parkinson's, CHF, COPD, and Arthritis
Close client cases via phone or home visits
Obtain online reviews from satisfied clients
Participate in community and networking events regularly
Requirements:
3+ years of outside sales experience in healthcare
Knowledge of the healthcare system and CRM tools (e.g., Hubspot)
Understanding of common senior diagnoses
Strong presentation and communication skills
Existing network or visibility in healthcare or home care community
Strategic, assertive, and customer-focused with excellent organizational and interpersonal skills
