Intake Department staff are the initial contact with Stratford VNA's referral sources and are responsible for initiating the intake, follow-through with the insurance authorization and assuring that patient care services are initiated and all appropriate individuals are informed throughout the process.

Position responsibilities include:

Receive the initial referral as per policy and through a variety of means and sources, assures that the proper information is obtained. Gather referral information electronically, orally and through written referral information received by the agency. Uses available resources to gather information as independently as possible. Identify patients who do not meet the criteria for admission per polices and makes appropriate referrals and recommendations for alternate services, consults with other senior managers in making this decision and demonstrates the proper knowledge of when this needs to occur. Assure that referrals are accepted on the basis of the agency's ability to provide the ordered care and services, both in terms of staffing and financially, through discussions with supervisory or other management members when necessary. Assure that proper and complete diagnoses are provided by the referral source and detailed as per CMS PDGM regulations. Assure referral inquiries are responded to as soon as possible to prevent loss of the referral. Assure that all orders for services, procedures and administration of medications are clear, legible, appropriate and complete. Medicare services must be ordered by a physician who has seen the patient within 90 days of referral or 30 days after start of care. Assure patients meet DPH and Medicare guidelines for homecare and the agency must be able to establish and maintain a safe plan of care. Prepare basic nursing orders to be followed by the primary care nurse at the time of admission. Obtain and receives all necessary insurance information and authorization. Obtain and records the name of a contact person and phone number from the referral source, and makes the initial contact with the insurer if the billing process is unclear in conjunction with the Billing Coordinator. Has clear knowledge and is responsible for keeping up to date on all insurances accepted by Stratford VNA. Work in conjunction with billing and authorization staff to provide accurate information to assure payment for services. Work in conjunction with the Care Transitions Coordinator to effectively communicate the status of hospitalized and SNF patients to obtain accurate patient and referral information. Assure accurate patient information is relayed to the next provider of care when patients are transferred to a facility, updates all disciplines involved and effectively tracks the status of transferred patients. Provide a complete report to the management team and Scheduler or designee on the patient's needs, admission date and patient location. Assure, together with the Therapy Scheduler and Supervisor of Clinical Services, that all ancillary services are ordered. Assist in identifying the needs for evaluation and services by others (e.g., social worker, physical therapist, etc.) Bring recommendations for education for referral sources or agency staff to the Supervisor of Clinical Services/Vice President of Patient Care Services as indicated to keep the intake/referral/ admission process going smoothly. Work with billing staff to resolve documentation related issues to assure proper and timely billing for services rendered and authorization for services rendered is received. Adapt to the needs of referral sources to ensure process to refer to Stratford VNA is efficient and deter referral sources from seeking other providers. QUALIFICATIONS

A current license to practice nursing in the State of Connecticut, an associates or baccalaureate degree in nursing and at least four years of full-time clinical experience in nursing preferred, at least one of which was in a home health agency. Generalized public health experience, under supervision, for at least two years. Complies with accepted professional standards and practice. Personal characteristics, personality and appearance appropriate for level of responsibility indicated. Personality which shows aptness in establishing and maintaining effective working relationship with administrative staff, staff members, representatives of other agencies and others. Possess excellent observation and communication skills. Is flexible and cooperative in fulfilling all obligations. Complies and maintains current CPR certification. Must be a licensed driver with an automobile that is insured in accordance with State and/or Agency requirements and is in good working order.
