VNA Alliance currently serves Crawford, Mercer, and portions of Venango county and offers skilled nursing, palliative care, and therapy services. In addition to the mentioned, VNA Alliance offers additional medical services and implemented a Safe at Home program that is a clinical pathway to optimize outcomes in the acute care and home health care setting. The Clinical Management Assistant supports patient care from admission to discharge while assisting the Clinical Manager and Executive Director with daily operations.
Key Responsibilities:
Run reports, maintain spreadsheets (Excel proficiency required), and assist with scheduling/meeting coordination Make patient calls to enhance experience Support onboarding, training, and special events Compile and review patient satisfaction survey data Document meeting minutes and provide backup support as needed Qualifications:
2+ years home health experience preferred Strong communication, organizational, and EMR skills Advanced Excel skills required Knowledge of agency policies, regulations, and payers Reliable transportation, valid driver's license, and insurance
