Job Overview

The role of the Service Coordinator is to assess, coordinate and provide linkages to community-based support services to adults age 18 and older who have a physical disability OR are age 65 or older at time of application for services OR have a diagnosis of Traumatic Brain Injury or related condition. The individuals must be assessed to need a nursing home level of care but choose to live in the community with supportive services; they must be enrolled in the NYS Medicaid program; and they must be eligible for either the NYS Traumatic Brain Injury Medicaid Waiver or NYS Nursing Home Transition or Diversion Medicaid Waiver programs. The Service Coordinator will help clients achieve their goals of living in the community by creating and maintaining service plans with direct client input; connecting clients with appropriate community services, programs and support systems; coordinating scheduling and tracking of services to ensure timely and accurate delivery; and advocating for clients to ensure they receive the services they need. This organized approach aims to address fragmented or complex needs by providing a central point of contact fostering independence and community integration of clients. The ideal candidate will have the ability to build strong relationships with clients and care partners; strong verbal and written communication skills; excellent organizational abilities to manage multiple schedules, records, multiple client cases and deadlines; and critical thinking and problem-solving skills to address challenges and find effective solutions.

Job Responsibilities Include But Are Not Limited To:

· Assess the service needs and wants of the client and facilitate their involvement in setting goals to establish and coordinate an individual service plan that links clients with needed supportive services.

· Provide client education to ensure that clients understand their overall program benefits as well as what they can expect the program and services to provide.

· Monitor and assure that the Service Plan is implemented timely, accurately and appropriately and support the client to become an effective self-advocate and problem solver with their care plan providers

· . Facilitate open communication with clients about their questions or concerns related to coverage and services supported by meeting with clients in their home quarterly and otherwise maintaining at least a monthly contact via phone, email, or Zoom etc. Driving up to 50 miles one way may be required. There is mileage reimbursement. (This is when seeing your client quarterly. Some individuals require more face-to-face encounters but that is on a case-by-case situation.)

· Review the individual service plan with the client routinely to ensure goals remain relevant and timely and to review progress.

· Maintain accurate and timely recordkeeping, reports, progress notes and other required documentation in the electronic medical record system.

· Provide recommendations for process improvement and services improvement within the agency aimed at improving client experience, work flow, work environment.

· Additional duties as assigned.

Qualifications

· Master's degree with one

(1)

year of experience providing case management/service coordination and information, linkages, and referrals regarding community-based services for individuals with disabilities and/or seniors.

· Bachelor's degree with one

(3)

year of experience providing case management/service coordination and information, linkages, and referrals regarding community-based services for individuals with disabilities and/or seniors.

· Associates degree with one

year of experience providing case management/service coordination and information, linkages, and referrals regarding community-based services for individuals with disabilities and/or seniors.

· Active Driver’s License.

About Guiding Light Resources Inc.:

Guiding Light Resources (GLR) was created in June 2020 and was authorized by NYS Department of Health as a Traumatic Brain Injury Medicaid Waiver Provider of Service Coordination and a Nursing Home Transition and Diversion Medicaid Waiver Provider of Service Coordination services. GLR provides Service Coordination to adults age 18 and older who have a physical disability OR are age 65 or older at time of application OR have a diagnosis of TBI or related condition. The individuals must be assessed to need a nursing home level of care but choose to live in the community with supportive services and they must be enrolled in the NYS Medicaid program. Service Coordinators assess clients and assist them in developing an individual service plan that helps the person reach their goals for living and receiving care plus support the person achieving their maximum independence in the community. Service Coordinators are responsible for providing linkages as well as coordinating and monitoring their clients’ supportive community services. Additionally, the Service Coordinator periodically meets with the individual to reassess their living situation; to clarify the current needs and goals they identify and with client collaboration, may revise their service plan to address changing circumstances. Current clients of GLR reside in Orange, Rockland, Dutchess, Sullivan, Putnam, Westchester or Ulster counties, or within New York City.

GLR is a highly respected Service Coordination agency with a reputation within the community and state for providing quality, effective services by clinicians who genuinely care about their clients. Demand for these services is very high. As a result, GLR is rapidly growing to best meet the needs of the community. Compensation:

Guiding Light Resources Inc provides salary ranges that comply with the New York Law on Salary Transparency in Job Advertisements. The salary range for the full- time role is $40,000.00- $56,000.00 annually. Actual salaries depend on a variety of factors, including experience, education, and operational need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.

The company offers PTO that is accumulated at the start of employment and may be used after the 90-day probationary period.

We offer a 401k which you are automatically enrolled in after one year of active employment.

We do not currently offer any kind of healthcare insurance.
