This is a variable shift position with options for:
8:30 am - 4:30 pm Four (10) hour shifts from 7:30 am - 5:00 pm No call is required, but weekend and holiday rotation is mandatory.
Job Description
We are seeking two experienced RN Care Managers for a 13-week assignment at our main campus in York, Maine. The ideal candidates will work 40 hours per week and will play a crucial role in coordinating concurrent utilization management activities throughout the facility. As an RN Care Manager, you will be the primary link between patients, the healthcare team, the organization, and payors during their hospital stay.
Key Responsibilities
Conduct assessments of patient needs and identify required resources for transitioning patients to the next setting. Collaborate, educate, and lead an interdisciplinary team of professionals in daily care rounds, care conferences, and care planning. Utilize excellent written, verbal, and electronic communication skills with software applications used within the hospital. Work with community organizations to secure durable medical equipment, health insurance authorizations, and transportation arrangements. Participate in the management of inpatient length of stay and understand applicable hospital reimbursement methods and quality measures. Qualifications
Associate or Bachelor degree-prepared Registered Nurse. 3-5 years of clinical experience in medical/surgical nursing, home care, hospital case management, or previous utilization review experience. Utilization review and discharge planning experience is required. Certification by a nationally recognized case management or utilization review organization is preferred. Active BLS certification is mandatory. CCM or ACM certification is preferred. Additional Information
