What We Are Looking For
The Supervisor, CMT Community Health is responsible for the day-to-day operational oversight of Care Management Community Health Workers(CHWs). The Supervisor will ensure positive patient experiences, effective coordination, efficient operations, and accurate data collection/reporting. Will participate in continuous process improvement practices and ensure adherence to policies and standards. Will provide training and development to staff. Responsible for time entry, performance measures, and staff disciplinary processes. This position will require frequent travel between clinical sites and patient-oriented locations.
Schedule : 5 days per week, 8 hours per day, Monday - Friday 7:30am-4pm

ESSENTIAL FUNCTIONS AND DELIVERABLES
A well-developed understanding of program guidelines is required. Responsible for overseeing day-to-day operations and program integrity for Community Health Worker staff.
Provide administrative and field-based supervision to Community Health Workers.
Participate in the evaluation, training, and development of staff.
Collaborate with program and department leadership to develop and maintain systems and processes for outcome management. Assist in identifying areas needing revision and participate in the practice of process improvement.
Mentor, guide, coach direct reports to expand their capabilities, increase productivity, and improve performance.
Collaborate with management on employee performance appraisal and goal setting.
Responsible for employee timecard entry and approval.
Monitor field-based patient visits for safety. Ensure documentation meets organizational expectations and monitor for events requiring mandated reporting or escalated follow-up.
As needed, perform field-based intensive case management for a caseload of at-risk patients, such as, but not limited to, homelessness, chronic medical, behavioral, substance use health conditions, social determinates of health issues, children and/or your, and individuals with post-incarceration.
Engage community agencies and build organizational relationships with community-based organizations and programs. This position will serve as the liaison between SAC Health System and the surrounding community.
Oversee the preparation and delivery of Systematic Case Reviews. Ensure that patients who present receive adequate follow-up and intervention.
Collaborate across departments to ensure Care Management workflows align with organizational expectations.
Assist with internal audits. Provide feedback to staff on program guidelines and workflows.
Assist the Quality Department with special projects, as needed. May contribute to the investigation of events and grievances and assist with patient relations.
Ability to be flexible to assist in all departmental areas when needed and travel to other clinics as necessary; must have a reliable vehicle, valid driver's license, and auto insurance.
Travel to clinic sites, patient homes, community services, and health centers/hospitals will be required.

QUALIFICATIONS:
Education : High School diploma or equivalent is required. AA in Social Work, or equivalent work experience in a medical/mental health setting preferred.
Licensure/Certification : Current CPR/BLS certification (must be American Heart Association or Red Cross accredited program), valid California driver's license, and auto insurance. As a requirement of this position, you must receive EPIC certification for the module you have been hired into. Community Health Worker certification is required.
Experience : 5+ years of experience in a community-based setting or related experience is required. 3+ years of experience working as Community Health Worker or related position.
Essential Technical/Motor Skills : Must be proficient in MS Office Suite (Word, Excel, PowerPoint, Outlook). Must be able to use widely support internet browsers. Must have the ability to use variations of electronic health records and other various databases.
Interpersonal Skill s: Able to relate and communicate positively, effectively, and professionally with others; be assertive and consistent in following and/or enforcing policies; work calmly and respond courteously when under pressure; lead, supervise, teach, and collaborate; accept direction. Able to communicate effectively in English in person, in writing, and on the telephone; think critically; perform basic math functions; manage multiple assignments effectively; compose written material; organize and prioritize workload; work well under pressure; problem solve; recall information with accuracy; pay close attention to detail; work independently with minimal supervision
Essential Mental Abilities : Must be flexible to perform a variety of tasks. Must be well organized and a self-starter. Must have strong analytical and problem-solving skills.
