Job Description

The LPN Medication Care Manager is responsible for providing the highest degree of quality care and services by administering medication and treatments in a safe organized manner. Responsibilities include administration of medications, documentation of medication administration, conducting SHUs, and providing resident care in accordance with Sunrise policies and applicable laws and regulations.

Responsibilities & Qualifications

Medication Administration

  • Provide the highest degree of quality care and services by administering medication and treatments in a safe organized manner.
  • Review, read, notate, and initial the electronic health record to document and learn about pertinent information about residents.
  • Receive medication updates from the Resident Care Director (RCD) or Wellness Nurse.
  • Administer, assist with, and observe medications and treatments for each resident using the medication administration record and the Six Rights of Medication Pass (Right resident, medication, dosage, time, route, right to refuse).
  • Ensure medications are passed according to times using a mobile medication cart.
  • Document and initial as medications are given and ensure appropriate documentation for refusals or missed doses.
  • Maintain confidentiality of all resident information including medications.
  • Report resident concerns and unavailable medication to the RCD or Wellness Nurse.
  • Restock medication cart after passes.
  • Assist in checking medication regardless of packaging system.
  • Assess residents to determine the need for as-needed medications and document/report to supervisor.
  • Count narcotics with another Medication Care Manager or Lead Care Manager each shift and report discrepancies.
  • Maintain and clean the Medication Room, medication carts, and treatment carts; ensure medications are available and not expired.
  • Follow re-fill process for medications.
  • Help residents maintain independence and promote dignity and safety in line with Sunrise Principles of Service.
  • Demonstrate empathy and adapt to each resident’s communication patterns and needs.
  • Practice standard care precautions for cleanliness, hygiene, and health.

Resident Care

  • Notify RCD of resident and/or family concerns.
  • Attend and participate in daily Cross Over meetings.
  • Conduct Service and Health Updates as directed by RCD.
  • Participate in the development of Individualized Service Plans (ISP).
  • Transcribe orders as required.

Risk Management and General Safety

  • Ensure community compliance with safety regulations and occupational health requirements.
  • Follow safety procedures including use of PPE, fire safety, SDS, and Lockout/Tagout where applicable.
  • Report accidents, incidents, and unsafe conditions immediately.
  • Keep cords, carts, and equipment orderly and unobstructed.
  • Comply with infection control practices and proper handling of hazardous materials.
  • Report occupational exposures promptly.
  • Store oxygen tanks safely and monitor prescribed levels.

Training and Team Contribution

  • Participate as a team member; support team goals and Sunrise Credo.
  • Engage in resident and guest services through Sunrise Principles of Service.
  • Contribute to engagement programs and training; attend regular meetings and trainings as directed.
  • May be designated as shift supervisor or supervise other Medication Care Managers.
  • Maintain compliance with required trainings and regulatory standards.
  • Perform other duties as assigned.

Experience And Qualifications

  • LPN, LVN, or state/province licensed nurse credential
  • Maintain required certifications where applicable
  • CPR and First Aid certification
  • Must be at least 18 years of age
  • Experience with seniors preferred
  • Desire to serve and care for seniors
  • If applicable, drive responsibilities per SunriseDriver requirements
