Community-based Mental Health Specialist
Assertive Community Treatment (ACT) Program
Full-time: Monday-Friday 9a-5p
The Charles Evans Center (CEC) is a leading not-for-profit located in Nassau County. CEC assists adults diagnosed with mental illness in reaching the highest level of independence with the freedom to ASPIRE HIGHER in their hopes, dreams and accomplishments. We are currently seeking a few exceptional people who have a passion to create a meaningful impact in the lives of the amazing people we support.
The Mental Health Specialist will provide direct community support to the recipients on the Mobile Residential Support Team
In this position, you will:
Ensure quality services are provided to each recipient through person-centered planning and maximize opportunities for integration within the community.
Collaborate with MRS staff members to collectively address the needs of recipients on the team.
Utilizes crisis intervention skills to ensure recipient’s safety in the community.
Implement and document the delivery of all community support services which are designed to help recipients effectively manage psychiatric symptoms, live independently, and successfully in their community of choice.
Establish mutually agreed upon support plan goals to enhance independent functioning in the community.
Implementation of all person-centered behavioral support plans/strategies and documentation of recipient behaviors.
Provide necessary assistance to recipients to promote the highest level of health, safety and independence.
Evaluate and support recipients where necessary in learning how to perform daily living tasks including, but not limited to, apartment cleaning and household chores.
Ensure that living environments meet the highest quality standards and utilize behavioral interventions as warranted.
Maintain collaborative relationships with other CEC programs, as well as outside providers, to maximize service coordination and integration.
Participate in on-call coverage.
We require:
Bachelor’s degree in Social or Human Services; Master’s degree preferred.
At least 6 months experience working with mental health population, required.
Clean, valid New York State driver’s license.
Must have transportation and be able to travel through Nassau and Western Suffolk County.

Unlicensed Bachelor's: $30.90
Unlicensed Master's:$32.25
Limited Permit: $33.30
Licensed LMSW: $37.30
Licensed LCSW or LMHC: $42.33
