Day Shift, 5x8-Hour (08:00 - 16:30) Job Responsibilities: Provide skilled physical therapy services to adult and geriatric patients in a home health setting. Perform evaluations, develop individualized treatment plans, and deliver therapy focused on functional mobility, strength, balance, and independence. Manage a caseload with up to 6 patient visits per day (30 productivity points). Collaborate with interdisciplinary team and educate patients and families on home exercise programs. Complete documentation using EPIC and OASIS systems. Travel within Medford and surrounding areas with occasional driving up to 50+ miles (mileage reimbursed at IRS standard). Required Skills & Experience: Minimum 1 year of recent

skilled home health experience

(required) Proficiency with

and

EPIC
