Position Summary

The Employee Health Nurse is responsible for implementation of the Employee Health program, processing and administering workers compensation, disability and FMLA claims as well as coordinating leave management system with carrier and providing a Wellness Program for employees.

Essential Job Functions: Maintains health records for all staff assuring that annual health assessment, physicals and tuberculosis (PPD - testing and reading) screening and Respirator Fit testing are current for Friendly Home, Linden Knoll, Cloverwood/Glenmere and Lily Brook. Schedules and performs new employee physicals and ensures that New York State standards are met. Along with onboarding of Volunteers and students. Completes and tracks Annual Health assessments on all employees. Provides education and vaccinations for Hepatitis B, COVID-19, annual Influenza and Pneumococcal to all interested staff. Organizes annual Vaccine clinics and program for staff. Provides counseling and health related information to staff as needed. Keeps abreast of changing regulations regarding workers compensation, OSHA, disability, American Disability Act (ADA), FMLA and anything else related to employee health and safety. Educates management and employees on topics related to workers compensation, disability, FMLA and safety in collaboration with HR. Implements, promotes and trains on employee wellness programs and initiatives. Serves as a member on the Workers Compensation and Safety Committees. Processes and administers Workers Compensation claims. When applicable, reports to scene of employee accident or incident, provides assessment of injury and first aid, makes necessary referrals for medical treatment. Completes incidents reports and when applicable report disability claims to carrier. Follows up with department heads and employees regarding questions on incidents reports and preventative safety education and plans Processes workers compensation claims with workers compensation carrier on a timely basis within regulations timeframe. Maintains and updates OSHA log and reports. Liaises between workers compensation carrier and employee. Tracks employees out of work on workers compensation - provides a weekly phone call to monitor employee's progress. Implements, administers and supports "Return to Work" program by corresponding with employees' to bring employees back to work on "transitional (light) duty" with proper documentation. Monitors employee's progress and makes recommendations on employee's status following Federal and State regulations. Updates transitional job descriptions and completes TWA paperwork. Works in partnership with HR, assisting in Leave Management process, includes NYS Short Term Disability, Family Medical Leave Act (FMLA) and Paid Family Leave (PFL) When applicable report DBL, FMLA and PFL requests to carrier. Follow up with employees on returning to work, ensuring Fitness for Duty forms and/or medical documentation received. Update Department heads and Leave management group of any return to work/ medical restrictions of employees on leave Works with the Infection Prevention Nurse and Medical Director to: Identify Persons Under Investigation (PUI) for COVID-19 and provides necessary guidance to employee regarding testing and return to work requirements Assists Infection Prevention in completing an outbreak investigation and targeted testing for employee outbreaks. Assists Infection Prevention with COVID-19 record-keeping as well as DOH and CDC reporting Follow up calls for staff call -ins related to infectious disease and tracking Performs other duties as assigned within Federal and State regulations. Other Job Functions:

Supports neighborhoods and assists as needed, including provide RN assessments in facility Track Employee Health data utilizing FSL Office software and excel spreadsheets. Provide data analysis to facility Quality team. Expectations of the Position: Dependable, report to work on-time for every schedule shift (ready to work at beginning - start of shift) and work full schedule shifts unless physically unable or pre-approved Paid Time Off (PTO) which includes personal and vacation. Willing and able to work as required by the position and/or approved by management. Have reliable transportation to and from work. Must be clean and neat in appearance and have a "friendly" service-oriented, team, cooperative demeanor and supports Culture of Excellence initiatives by interactions with co-workers and/or Members. Abide by work-related / job procedures and organization policies; (refer to the Employee Handbook). Qualifications of the Position:

Education: R.N /B.S.N. preferred. Experience: At least five years of progressive nursing experience. Skills: Ability to travel between Friendly Home and Cloverwood. Ability to meet the public and participate in professional programs. Licenses / Certifications: Current New York State Registration and License. Conditions of Employment:

Work is performed in a clean, well lit, "Home-Like," nursing Home setting. Employee must be able to perform essential job functions (reasonable accommodation may apply). This is a level 3 category. Refer to Physical Requirements form. Must meet the general health requirements set forth by Friendly Home. May be requested to assist in the evacuation of Members or perform other procedures to protect the safety of Members, visitors and team members during emergency situations.

Pay Range:

$33.03 - $39.33 per hour
