Summary :
The Youth Services Support Program Coordinator supports the operational and administrative functions of the Youth and Family TREE Program, a federally funded initiative designed to enhance and expand early intervention, treatment, and recovery support services for adolescents, transitional-aged youth, and their families affected by substance use disorders (SUD) and co-occurring mental health conditions. As a key personnel program position, the Youth Services Support Program Coordinator will work in collaboration with the Program Director, coordinating services and activities, including training, communication, and the dissemination of information.
This position will be responsible for data collection and accuracy, collaboration and coordination with community partners, program quality assurance and improvement, and maintaining grant-required program fidelity. The Youth Services Support Coordinator provides excellent customer service to patients and providers, while ensuring access to care, assessment for additional services if qualified, and coordination of non-traditional healthcare services to ensure whole patient care is achieved.

Essential Job Responsibilities: Represent Alluvion Health in all Youth and Family TREE program activities and community outreach efforts. Coordinate daily program operations, including scheduling, documentation, and team communications to support seamless program delivery. Work closely with the Project Director and other staff members to ensure alignment and coordination with other initiatives, including behavioral health, primary care, and SUD services. Serve as a liaison between TREE program participants and internal/external service providers to ensure access and warm handoffs to care. Collect, enter, and maintain accurate participant and program data in alignment with grant reporting, quality assurance, and compliance requirements. Facilitate in-clinic referrals for other Alluvion Health services, such as dental, outreach/enrollment, and prescription assistance, as appropriate. Support patient and youth engagement and ensure follow-up in care through evidence-based practices and coordination with clinical and community teams. Assist in the development, review, and maintenance of program policies, procedures, outreach materials, and internal workflows that reflect program goals and compliance standards. Collaborate with internal departments and external stakeholders to coordinate youth services, training events, and activities related to substance use prevention, treatment, and recovery. Help plan, organize, and attend youth-centered events, meetings, and coalition activities to foster community awareness and involvement in TREE initiatives. Create and deliver presentations, informational handouts, and outreach materials; assist with managing social media and public messaging. Attend local events, meetings, and tabling opportunities to support outreach, survey efforts, and community-based engagement strategies. Participate in team meetings, quality improvement activities, as required. Maintain HIPAA compliance and uphold confidentiality in all youth and family interactions. Monitor and support quality of care and fidelity of TREE services, providing feedback for program improvement and participating in audits and internal reviews. Perform various special projects and duties as assigned, which may include research, data collection, participation in cross-agency meetings, or representing the program.

Education/Experience/Qualifications:
Education/Training:
Three (3) to five (5) years related experience required; Associates or Bachelor's Degree in Social Work preferred; Experience in a Federally Qualified Health Center or Healthcare Setting preferred. Certifications:
Certified Application Counselor and/or SHIP certification experience preferred; Must possess a valid driver's license issued by the state of Montana. Language Skills:
Fluent in the English language. Ability to read, analyze, and interpret the most complex documents. Ability to respond effectively to the most sensitive inquiries or complaints. Physical Requirements:
