At NorthBay Health the Clinical Pharmacist is responsible for providing the clinical judgment, professional knowledge and distribution skills in the Pharmacy Department to ensure that patients receive pharmaceutical care which is consistent with the goals of the Department and the Organization.

Qualifications

PRIMARY JOB DUTIES

1.

Reviews drug orders, patient profiles and medical records to evaluate the appropriateness of drug therapy.

2. Selects, compounds, prepares and dispenses drugs in accordance with departmental procedures, all laws and regulations.

3. Oversees and directs shift workflow and departmental aspects of workflow performed by technicians and interns.

4. Consults with members of the medical staff, nursing staff, patients or other hospital personnel regarding all aspects of drug therapy and serves as a resource for drug information.

5. Maintains and ensures accuracy and completeness of patient profiles according to departmental procedures. Routinely performs pharmacy profile-nursing medication administration record (MAR) comparisons.

6. Documents all clinical interventions and formulary compliance/noncompliance with the medical staff in the Electronic Health Record.

7. Identifies and reports adverse drug reactions (ADRs) and medication errors according to established departmental policy and procedure.

8. Under specific medical staff approved protocols, a pharmacist may initiate and/or adjust drug

JOB SPECIFICATIONS

1. Education: Doctor of Pharmacy (Pharm D.) or Bachelor of Science Degree in Pharmacy, and evidence of required continuing education.

2. Licensure/Certification: California Board of Pharmacy licensure.

3. Experience: One year of hospital-based experience in unit-dose distribution and IV additives and/or hospital residency required.

4. Skills: Excellent verbal and written communication skills with drug information and therapeutics. Ability to apply professional knowledge and judgment to the provision of pharmaceutical care for patients of the following age groups: Geriatric, Adult, Pediatric and Neonatal.

5. Interpersonal Skills: Demonstrates the True North values. 7. Physical Effort: Attendance is an essential function of the job. The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. 8. Hours of Work: Various shifts.

Compensation: Hourly Salary Range MIN $93.00- MAX $111.00 (Offered hourly rate based on years of experience) Per Diem differential 10% included in the hourly pay rates PM Shift Differential $6.60 per hour NOC Shift Differential $21.00 per hour
