Your Role and Responsibilities Perform in-depth patient evaluations, order necessary diagnostic tests, and create personalized care plans Diagnose and manage acute or chronic health conditions independently or collaboratively (depending on credentials) Provide education to families and patients on wellness, prevention, and chronic disease management Prescribe medications and treatments within your scope of practice Engage in chronic care management and patient follow-up Work alongside interdisciplinary teams to ensure integrated and comprehensive patient care Maintain accurate and complete documentation in electronic medical records (EMR) Educational Qualifications

Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) Completion of an accredited Nurse Practitioner training program Mandatory Certifications

Active Nurse Practitioner license in Colorado National certification as a Family Nurse Practitioner (FNP-C, AANP, or ANCC) or equivalent specialty Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications (or willingness to obtain within 90 days) Essential Skills for Success
