The Wellness Center itself will be a state-of-the-art facility providing comprehensive, affordable healthcare services, including medical, dental, and behavioral health, to patients of all ages, from newborns to seniors.

Erie Family Health Centers, a nationally recognized top workplace with 13 sites in Chicago and suburbs, is looking for a valuable addition to our Case Management Department! The Birthing Families Navigator works with medical providers and care teams to identify potential clients, assist, and deliver supportive services to clients. The Birthing Families Navigator's role includes outreach, advocacy, assessments, care plans, referrals, home visitation as needed, and documentation in funder data management system and electronic health record.

*Compensation is based on each candidate's experience, skills and education within the range identified for the role. Candidates who meet the minimum requirements of the role will start at entry in the range. Any additional skills, experience and education will be reflected in the compensation offered.

Main Duties & Responsibilities Provides services including coordination of primary care (prenatal, post-partum, and/or pediatric care) and social services, outreach, face-to-face follow ups, risk assessments completion, referrals, home visitation as needed, and track clients' appointments failures. Collaborates with fellow Birthing Families Navigators to provide constant floor coverage so as to ensure productive presence of a Birthing Families Navigator throughout the day. Collaborates with client's provider and care team. Displays familiarity and knowledge of the care plan of each client and facilitates referrals and follow-up process. Displays excellence in customer relations. Completes monthly reports as assigned. Qualifications

Education

Required:

Associate's degree in social work, child development, community health, or related field required. Preferred:

Bachelor's degree in social work, child development, community health, or related field preferred. Experience:

Preferred:

A minimum of two years' work experience in health, social or community services preferred. Skills and Knowledge

Required:

Intermediate proficiency in Microsoft Office required. Bilingual fluency (English/Spanish) is required depending on the Erie site and/or population served. (May be required to assist or work with non-English/Spanish-only speaking patients and/or guests.) Local travel required, valid Illinois driver's license and automobile insurance also required. Preferred:

Cultural competency to work in a low income, minority environment. Ability to maintain confidentiality and trust from clients. Ability to establish and maintain effective working relationships with customers, clients, and community partners. Ability to work well individually and as part of a team.
