Come join our team and love what you do! We are currently hiring a full-time Intake Coordinator - LVN/LPT for Fremont Hospital. Evening shift available: PM (3pm - 11:30pm). As an active member of the Assessment & Referral Team, the Intake Coordinator - LVN/LPT performs the Initial Psychiatric Screening Assessments for incoming patients and coordinates with the Medical Staff in directing the patient towards an appropriate level of care. The facility provides a full assortment of care which includes assessments, inpatient psychiatric hospitalization, partial-hospitalization, intensive outpatient psychiatric treatment as well as chemical dependency outpatient care. Qualifications
Education: AA/AS Degree or Bachelor's Level Degree preferred; Master's in social work, psychology or related mental health field, PhD and PsyD candidates in terminal degree programs with Masters equivalent or a degree from an accredited program in nursing preferred. Experience: A minimum of one (1) year direct clinical experience in a psychiatric or mental health setting, with direct experience in patient assessment, family motivation, treatment planning, and communication with external review organizations. Licensure: LVN or LPT licensure required.
