Patient Care Coordinator
The Patient Care Coordinator (PCC) is responsible to coordinate the delivery of medical care, as formulated by Northwest Weight & Wellness Center (NWWC) physicians or their designee, and the patient's access to various health care services. The PCC will coordinate the patient's entry into and through the patient journey, coordinate patient care services and serve as patient advocate. Job Responsibilities: Works independently under the general direction of a physician or their designee. Acts as primary liaison of NWWC and represents departmental physician(s) with the hospital, patient, and family, referring physicians, outside agencies, staff from outside hospitals and general public by providing information and advice regarding NWWC to coordinate patient appointments, admission, or surgery before and after patients are seen. Communicates with a variety of Health Care Professionals, patients and their advocates, hospital, and clinic personnel both at NWWC, representatives of insurance companies and other organizations related to the health care industry. Obtains all necessary documents and coordinates the patient's entry into the system to include the availability of necessary medical records, diagnostic studies, pathology items and coordination of tests, procedures, and appointments with all hospital departments. Requests interpreters as appropriate for appointments, admissions, and surgery. Maintains knowledge of billing and medical record documentation requirements, diagnostic tests and procedures and specialized pre-procedure instructions with guidelines set by clinic leadership and clinic Medical Director. Answer the telephone in a professional and courteous manner and directs calls appropriately. Responsible for reviewing registration data such as demographic, insurance, referral, next of kin etc., and updating online or providing information to appropriate personnel so that an update may be done. Schedule patient appointments by following the appropriate guidelines by clinic, department, and physician. Enter directly into appointment system or triage to appropriate person. Responsible for completing required hospital and Sound Surgeons Surgery Center (SSSC) documentation Responsible for reviewing registration data with patient for scheduled inpatient surgery/admission or outpatient surgery/procedures. E.g., demographic, insurance, referral, next of kin etc., and updating online or providing information to appropriate personnel so that an update may be done. Responsible for coordination of pre-admission tests and procedures. Communicates cancellations or changes regarding patient procedures to the appropriate team members for continuity of care. Acts as liaison between patient and healthcare team members for the purpose of coordinating all pre-op planning and preparation. May occasionally schedule and be required to communicate pre-op planning and preparation to patient. Sends appointment and other information to patients. Responsible for working with the surgical services leadership for ensuring schedule accuracy to allow for efficient use of resources in the Surgery Center and management of surgeon block time. May assist with completion of surgery packet. Responsible for the tracking and coordination of surgical intervention- Ensuring all required steps for patient surgical approval and readiness are met by the patient and healthcare organization. Supervisory Responsibilities: N/A Schedule: Hourly/Non-Exempt Schedule varies, Monday-Friday, 8am-5pm Education/Experience Requirements: High School Graduate/GED equivalent. Two (2) years' experience in a tertiary or Ambulatory Surgery Center (ASC) patient care setting. Certified Nursing Assistant (CNA) Medical Assistant (CMA) or Licensed Practical Nurse (LPN) preferred. In place of the above requirements, the incumbent may possess any combination of relevant education and experience which would demonstrate the individual's knowledge, skill, and ability to perform the essential duties and responsibilities listed above. Physical Requirements: Must be able to sit, stand, walk, bend, squat constantly; reach with hands and arms above shoulders occasionally; stoop, kneel or crouch seldomly. Lift/Carry/Push/Pull up to 25 lbs. occasionally. Must possess functional vision, hearing, and speech to communicate effectively with clients, client advocates, and staff. Performs manual dexterity, fine motor skills and repetitive hand motions. Specific vision abilities required by this job include close vision and ability to adjust focus.
