The Patient Safety Coordinator is responsible for ensuring the safety and well-being of patients throughout the healthcare facility. This role involves identifying and minimizing risks to patient safety, coordinating safety initiatives, and ensuring compliance with safety standards. The Patient Safety Coordinator works closely with clinical and administrative teams to promote a culture of patient safety, enhance the quality of care, and support key patient safety initiatives. Responsibilities include collecting and analyzing data, maintaining relevant databases, generating and interpreting reports, and ensuring that the appropriate Directors respond promptly to safety investigations. Review safety event entries, ensuring timely follow-up with appropriate staff, and refer to Medical Staff Services as necessary. Coordinate and contribute to the collection, analysis, and reporting of data supporting performance improvement and patient safety initiatives, including data from safety event reporting systems. Participate in data collection and reporting to Solutions for Patient Safety and facilitate improvement efforts based on these findings. Lead Root Cause Analyses (RCA) and Apparent Cause Analyses (ACA), as well as performance improvement initiatives to evaluate risks, prevent errors, and identify opportunities for safety enhancements. Proactively identify and report issues, patterns, or trends that could lead to compensatory or sentinel events, such as Potentially Preventable Complications and Preventable Adverse Events, and work with leadership to implement corrective actions when necessary. Present and coordinate periodic in-service training for employees and medical staff on patient safety topics and safety event data. Develop and participate in educational programs aimed at reducing or eliminating safety hazards, in collaboration with the clinical education department. Ensure training is provided to employees, medical staff, and volunteers on identifying and reporting patient safety issues during orientation, annually, or as needed. Engage in the Child Health (PSO) initiative to stay informed about national safety trends and share relevant safety events from the institution. Promote a culture of patient safety by supporting and advancing patient safety programs across the organization. Occasional travel may be required for training, audits, or conferences. Must have reliable transportation. Education and/or Experience: Any combination of education and experience that would likely provide the required knowledge, skills, and abilities is qualifying. Typically has 6+ years professional experience. Healthcare Quality experience required. Bachelor's degree preferred. Knowledge: Strong understanding of healthcare regulations, patient safety standards, and risk management strategies. Excellent communication and interpersonal skills to collaborate effectively with staff, leadership, and external regulatory bodies. Responsibilities: Established own procedures with little supervision. Ability to handle sensitive and confidential information with integrity and professionalism. Certificates, Licenses, Registrations: Must have a clear unrestricted driver's license.
