Physical Therapistsat VNH screen, evaluate, re-evaluate, and treat patients as well as coordinate and communicate treatment activities with our interdisciplinary team members and the patient.

The PT will:

Evaluate the patient's need for physical therapy and develop an appropriate treatment plan.

Carry out appropriate treatment programs in accordance with agency policies and professional standards of care.

Communicate with all members of the health care team.

Document evaluations, treatment goals, and objectives.

Teach patients and family members the rationale for treatments and procedures.

Minimum Qualifications:

BS in Physical Therapy

Physical Therapist License in both VT and NHorhold a compact license

One year of experience as a PT in a home healthcare setting

Salary Range (depending on experience):

Minimum: $39.25

Maximum: $60.84
