Job Description Job Title: Physical Therapist Profession: Rehabilitation Therapy Specialty: Physical Therapist Duration: 13 weeks Shift: Monday to Friday, Days Hours per Shift: 32 hours guaranteed Experience: New graduates are welcome. SNF experience preferred. License: Valid state license required. Certifications: Proof of recent Covid test required 72 hours prior to start date.
Must-Have: - Completed prescreen form attached upon submission. - Resume and license verification attached upon submission. - References (3) provided. - County criminal background check for 7 years for all counties lived and worked. - Federal criminal background check for 7 years or more. - Multi-state criminal search for 7 years or more. - Nationwide sex offender search for 7 years or more. - HHS/OIG search completed. - GSA search reviewed. - Social Security number trace completed. - One-step PPD or Quantiferon test or proof of positive PPD and chest X-ray. - SAM search completed. - CORI background check completed. - 8 hours of dementia training completed.
Description: Ensure the prescreen form is filled out and attached upon submission. A resume and license verification need to be attached. If these items are not completed and attached, the candidate will be declined. Covid testing must be completed 72 hours prior to the start date, with results issued 48 hours prior.
