What You'll Do in This Role:
We are seeking a compassionate and dependable
Home Health Concierge
to join our
Program of All-Inclusive Care for the Elderly (PACE)
team. This non-certified position focuses on delivering individualized, participant-centered, non-clinical support that promotes safety, comfort, and dignity. Services are provided in participants' homes or in contracted settings such as Adult Foster Care, Assisted Living, or Skilled Nursing Facilities
Primary Responsibilities:
Provide non-clinical assistance such as companionship, light housekeeping, errands, meal preparation, and other supportive tasks as outlined by the Interdisciplinary Team (IDT). Support the participant's well-being and help maintain a safe and stable environment across various care settings. Build trusting, respectful relationships with participants and respond empathetically to their needs. Communicate observations or concerns to appropriate team members to support participant safety and care planning. 1:1 participant Work across
multiple care settings
based on participant needs and care plans. Be an important extension of the care team, even without a clinical license. Skills & Knowledge
Passion for working in a
collaborative, team-based model of care . Exceptional
communication
and
customer service
skills. Highly organized , with excellent
time management
and
prioritization
abilities. Professional demeanor
with strong
critical thinking ,
creative problem-solving , and
adaptability . Commitment to maintaining
high ethical standards

in all work. Self-motivated
and comfortable working
independently
in varied care environments. •
What You'll Need:
High school diploma or GED
required. Reliable transportation
and a
valid Michigan driver's license . At least
one year of experience
working with the
frail elderly
or in
long-term care settings . Possess current CPR certification or willing and able to obtain as offered by LifeCircles.
