Description Conduct assessments for patients in both inpatient and outpatient settings, creating personalized treatment strategies Implement physical therapy techniques focused on enhancing mobility, strength, balance, and overall function Work collaboratively with nurses, physicians, case managers, and other rehabilitation experts to maximize patient outcomes Provide education to patients and their families regarding therapeutic objectives, discharge processes, and home exercise regimens Ensure thorough documentation of evaluations, treatments, and patient progress according to facility and regulatory standards Engage in team care rounds and interdisciplinary meetings to maintain coordinated patient care Education

Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution Certifications

State licensure as a

Physical Therapist CPR certification preferred Specialized training (such as geriatric or neurological certifications) is a bonus, but not mandatory Skills

Preferably previous experience in a similar role Capability to manage patients with multifaceted medical and mobility challenges Excellent communication and teamwork skills Proficiency with digital charting and clinical documentation systems Benefits
