Job Description:
Palliative Care Advanced Practice Provider (APP)
Lahey Hospital & Medical Center (LHMC) is looking for a full-time
Advanced Practice Provider (NP or PA)
to join our growing Palliative Care and Geriatric Medicine team.
About the Role
You'll provide comprehensive
inpatient palliative care services
within our 350-bed academic medical center, including our ICU and transplant units. There may also be
outpatient opportunities
based on your interests.
You'll be part of an
interdisciplinary team
(physicians, social worker, pharmacist) and help educate fellows in our ACGME-accredited fellowship program. We're affiliated with UMass Chan Medical School, so you may be eligible for a faculty appointment based on teaching activities.
What You'll Do
Evaluate and address patient needs: pain/symptom management, goal setting, treatment options, advanced care planning, and end-of-life discussions.
Educate patients and families to support them during illness.
Contribute to the division's educational mission.
Work effectively with other healthcare professionals.
Consult with palliative care physicians as needed.
Inform the consulting team of recommended interventions and collaborate with providers.
Attend and participate in divisional meetings.
Maintain accurate patient records.
Help develop clinical practice guidelines for quality palliative care.
Participate in the division's on-call rotation.
Qualifications
5+ years
of clinical experience.
ACHPN
preferred.
Clinical experience in:
geriatrics, oncology, cardiology, pulmonary, nephrology, or neurology.
Massachusetts licensure.
Active DEA registration.
