is currently seeking experienced Board Eligible / Board Certified
Family Medicine or Internal Medicine physicians
to join our
Transitional Care Clinic
team in
Fort Wayne, Indiana .
Specifics of The Role Part-Time role - 20 hours per week to include rounds Monday - Friday Involves travel by conducting daily rounding on Parkview patients in high performing network skilled nursing facilities Nurse practitioners cover call; physician would need to be available as a backup by phone The Team
Group consists of
2 physicians & 5 advanced practice providers
that provide quality continuation of care to patients that are discharged from the hospital Comprehensive care team and clinical support model comprised of medical assistants and registered nurses Parkview Health
