Job Summary:

Provides clinical management and coordination of care for adult transplant patients in either the pre or post phases of transplantation. Interacts with all necessary disciplines to ensure the coordination of patient services across the continuum of care to facilitate placement on the national transplant waiting list. Provides unit specific leadership and serves as a role model in all areas of measurement. The first point of contact in an attempt to resolve patient/family needs. Staffs in Transplant Coordinator role 80% of scheduled time. Assists the Transplant Patient Care Services Manager and QA/Compliance Manager in quality initiatives within Transplant.

EDUCATION DESCRIPTION:

Associate's degree in nursing with a minimum of 10 yrs. of relevant experience is required; or a Bachelor's degree in nursing with a minimum of 3 yrs. of experience as a transplant coordinator. Bachelor's degree in nursing is preferred.

TRAINING DESCRIPTION:

EPIC superuser.

LICENSURE DESCRIPTION:

Requires current State of Wisconsin Registered Nurse License or a Multi-state Nursing License from a participating state in the NLC. Certified Clinical Transplant Coordinator (CCTC) certification, Certified Clinical Transplant Nurse (CCTN), or Acute/Critical Care Nurse (CCRN) is preferred.

SPECIAL SKILLS DESCRIPTION:

Intermediate to expert knowledge of the federal regulations (CMS, OPTN, UNET) regulations governing transplant nursing processes. The Lead Transplant Coordinator must have exemplary customer service skills.
