Position Summary:
The Community Health Worker builds trusting relationships with clients and their families while providing culturally appropriate support, education, and advocacy. This role focuses on connecting individuals to health and social services, conducting outreach, care coordination, and promoting community engagement. The ideal candidate will demonstrate strong interpersonal skills, adaptability, and a commitment to serving diverse populations in a collaborative and respectful manner. This is a 10-month position beginning in August 2025.
Key Responsibilities:
To provide culturally appropriate health education, information, and outreach in community-based settings.
To bridge/culturally mediate between individuals, communities and health & human services including actively building individual and community capacity.
To assist clients to access the services they need.
To provide direct services such as information counseling, social support, care coordination and health screenings.
To advocate for individual and community needs.
To understand the population and communities being served and conduct supportive research to optimize services.
Qualifications:
Required:
Education: High School diploma or equivalent required. Successful completion of a CHW training program preferred.
Experience: 1 to 2 years of experience working in a community-based setting.
Special Aptitudes/Activities: Bilingual (English/Spanish) required. Excellent oral and written communication skills; ability to establish positive, supportive and trusting relationships with patients and colleagues and to work collaboratively and effectively within a team; knowledge of community social services and resources; knowledge of medical terminology and basic computer skills required. Extensive understanding of the Holyoke community including community resources.
Preferred:
CHW certification preferred

Salary Range: $19.25 - $25.25
Based on years of experience, level of education, specialty and licensure/certifications. Additional Compensation for advanced education for qualifying positions.
