Job Summary
This position is responsible for providing care coordination to members via telephonic and/or field-based care management. The role involves informing and educating members on health care programs, engaging them in discussions about adherence to health goals, responding to inquiries, and supporting clinical operations with provider and member activities. Duties include completing health screenings, assessments, and care planning, which may require home or facility visits for complex and specialized populations. Required Job Qualifications:
Registered Nurse (RN) with 2 years of clinical care experience or licensed professional counselors, social workers, or therapists with similar experience and licensure as specified. Current, valid, unrestricted license in the state of operation, with compliance to reciprocity or license transfer requirements. At least 3 years of wellness or managed care experience involving clinical issues with members/physicians. Knowledge of the health and wellness marketplace and employer trends. Excellent verbal and written communication skills. Analytical skills including medical data analysis. Proficiency in Word, Excel, PowerPoint, and web-based applications. Valid driver's license, transportation, and willingness to travel within the assigned territory. Preferred Job Qualifications:
Fluency in English and Spanish. Three years of direct clinical experience. Experience with SNP plans (Medicare & Medicaid). Patient education and condition management experience. Management of complex or catastrophic cases. Relevant certifications in case management, project management, or healthcare.
