Job Summary

The Medical Program Clinical Manager supports the clinical offices by providing routine, direct patient care using evidence-based nursing practice. Acts as a clinical manager on the patient care team to provide care with compassion, competence, and commitment.

Additionally, the Medical Program Clinical Manager supports the medical program director(s) and the medical in providing and coordinating excellent quality care for patients within the clinic model.

Communication

Good oral and written communication skills are required for this position. Information must be exchanged using tact and persuasion appropriately. The person in this position must be able to communicate with the patients, staff, and others; and contribute to a positive environment.

Duties and Responsibilities

Efficiently perform the duties of adult and pediatric patient care such as patient triage, vitals, prior authorizations, venipuncture, medication administration (injections, nebulizer treatments, oral medications), lab order entry and preparation, patient education, prevention and follow up calls.

Provide clinical coverage by performing patient care in collaboration with multiple providers, including medical , and in different departments (including adult medicine, pediatric medicine, laboratory, and front desk) as needed.

Supports excellent quality, timeliness, and patient experience throughout all patient interactions within the medical clinic or other departments as needed.

Key player in the day-to-day operations of the internal medicine program under the direction of the Medical Program Director, Continuous Improvement Coordinator, and Clinical Office Manager. Includes planning and coordinating the work assignments of program clinical staff and collaborating as necessary with managers supervising non-clinical staff (i.e., front desk, Pharmacy, social services.)

Demonstrates and guides employee compliance with policies and procedures promoting patient care with high levels of organizational efficiency, communication, and safety.

Assist with all outstanding tasks and documents management (medication refill requests, lab and imaging reports, medical records) in collaboration with and as directed by the provider.

Demonstrates critical thinking and organizational skills, leadership, and appropriate role modeling for office staff.

Perform other procedures directed by the Supervising Provider, Clinical Office Manager, or other Clinical Support Leaders such as:

In-house waived testing; tests, Hemoglobin, glucometer, urine dip, hemoccult-stool, and throat swab

Diagnostics such as EKG or Blood Glucose

Routine vaccinations, medications, or injections

Set up and assist providers with GYN exams, rectal/STI exams, and other small outpatient procedures.

Performs accurate charting, patient contact & updates in EMR in accordance with WFHC policies and procedures.

Maintain facility stock medication and vaccines per organization procedures.

Assist patients to complete applications for pharmaceutical drug assistance programs.

Complete prior authorization process for medications or specialty services

Communicate with pharmacies and ADAP to monitor patients’ adherence to therapy.

Monitor and ensure the pre-operative clearance process is completed as requested.

Process prescriptions and refill requests as directed by the provider.

Performs other duties and responsibilities as assigned.

May occasionally need to travel from one clinic to another for coverage.

Credentials

Current, unrestricted LPN or RN license, issued by the state of Florida.

Knowledge

At least three years of related work experience required: must have knowledge in promoting wellness; management of chronic diseases/conditions within the primary care setting; training in HIV/AIDS is recommended; experience working within a clinic is .

Knowledge of Infection Control Practices

Proficient in Electronic Health Records and Microsoft Outlook

Physical Requirements

Level 2 background clearance required.

Proof of immunizations for (1) Tdap, (2) MMR’s, HEP B vaccination, and either a history of chicken pox (varicella), or documentation of the varicella vaccine, a PPD Test is done once upon hire.
