Job Summary:
In accordance with the mission and goals established by Independence Health System, provides diabetes self-management training, insulin pump training, and medical nutritional therapy to outpatients of the Diabetes & Health Management Center. The Certified Diabetes Educator (CDE) serves as a resource within the ADA accredited program for outpatient education. Works collaboratively with interdisciplinary team through ongoing collaboration, case consultation, and process improvement to enhance quality outcomes across the continuum of care. Assesses and evaluates individuals with diabetes to identify knowledge deficits and provide education using motivational interviewing techniques to promote behavior change for patients to manage the disease. Establishes and maintains relationships with other community agencies for referral and networking purposes.
Education:
Minimum: Bachelor Degree in Nutrition or Graduate of a Registered Nurse Program
Preferred: Master Degree in Food and Nutrition
Registration/Certification/Licensure:
Current ADA registration through the Commission of Dietetic Registration (CDR)
Current PA LDN license OR Current PA RN license
Current certification as a diabetes educator by the Certification Board of Diabetes Care and Education Specialists (CBDCE) (must obtain within 9 months of hire and maintain certification)
Insulin Pump Therapy Certified (must obtain within 9 months of hire and maintain certification)
Family & Friends CPR
Experience:
Minimum: None
Preferred: Prior experience with standards of care for Diabetes. Knowledgeable in diabetes technology including insulin pump therapy and continuous glucose monitoring.
Specific Job Responsibilities:
Provides Diabetes Self-Management education to patients/support system through traditional face to face patient education, group class, and the use of video conferencing.
Provide insulin pump therapy education to patients on behalf of major insulin pump manufacturers.
Serves as a resource to staff, providing education where and when necessary to providers, clinical staff, and patients.
Provides timely response to patients and families within scope of practice; consults with and refers patients to other healthcare providers as required to triage and address emergency, specific and/or changing needs.
Maintains knowledge of diabetes medications including insulin titration, problem solving, and guidance.
Assists department in maintaining service excellence and other duties as assigned.

Act 33 PA Child Abuse History Clearance
Act 34-PA Criminal Record Check from the PA State Police system
Act 73 FBI Clearance
Position Type/Expected Hours of Work:
Incumbent will be scheduled based on operational need
Travel may be expected locally between System locations.
