Description

Siouxland Community Health Center has an opening for:

Behavioral Health Case Manager

1– Full-time

Successful candidate must have solid experience & advanced knowledge of:

S.W. or appropriate Bachelors level degree.

Current BLS and Mandatory Reporter certifications

Minimum of two years experience in case management or related experience.

Understanding of socio-economic, culture, and other barriers to medical care and ability to diminish.

Knowledge of community resources and willingness to develop relationships with external agencies.

Successful candidate must be able to perform primary functions of position:

Educate and motivate patients on prevention and self management of mental health and substance abuse issues.

Formalize case plans, strategies, and summaries into written form.

Data entry and reporting monthly Quality and Clinical measures.

Assist providers in patient care as directed.

Identify why patients are not meeting Clinical goals and coordinate goal-achievement plans.
