RN Nurse Case Manager Responsibilities:
provide compassionate care that addresses physical, emotional and spiritual needs of patients and their families
supervise the day to day activities of assigned residents and employees
coordinate, plan, implement, and evaluate the nursing services provided to each resident using clinical competence
review and audit compliance with the patient's plan of care and changing needs
ensure appropriate staffing patterns are in place
evaluate employee duties and needs
hold employees accountable for following policies and procedures
provide a safe, comfortable, and home-like environment
on-call rotation
other duties as assigned
Required Qualifications:
current licensure in the State of Wisconsin as a registered nurse
degree from an accredited school of nursing
previous experience in related field
ability to prioritize duties in a constantly changing environment
effective management skills
customer service oriented with patients, employees, families, and other agencies
good problem solving skills
excellent time management skills
ability to accept and give directions
work well in a team environment
a desire to make a difference in people's lives
