Durham, NC . This full-time role is perfect for PTs who want to make a direct impact in
post-acute care, geriatric rehabilitation, and long-term mobility improvement .
You'll work alongside a supportive therapy department to deliver individualized
treatment planning , provide
Medicare/Medicaid documentation , and collaborate with PTAs and other healthcare professionals to ensure continuity of care.
Responsibilities:
Provide high-quality physical therapy services in compliance with SNF and Medicare guidelines Develop and implement individualized care plans Educate patients and caregivers on safe mobility and fall prevention Track progress and update treatment plans as needed
Requirements:
Licensed PT in North Carolina Graduate of an accredited PT program (DPT or equivalent) SNF or geriatric rehab experience preferred Strong documentation and communication skills
