Work hours: Full time; Tuesday - Saturday

Here's What You Need

H.S. Diploma or Equivalent required

Three years' relevant experience, preferably in a healthcare or service environment required

Patient Care/ Customer Service / Healthcare experience strongly preferred

What You Will Do

Responsible for coordinating the clerical and administrative processes for entity care transitions management

Responsible for managing deparmental mail box, phone lines and/or fax

Supports transition planning activities as needed under the direction of care transition managers

Calls vendors, physician offices, clinics, etc

Faxes (electronic and traditional) documents as needed

Makes copies or gathers other data as needed

Collect and enter data as needed

Completes other tasks and projects assigned
