Position Overview
The Oncology Nurse Navigator serves as a dedicated care coordinator throughout the cancer care continuum. This role assesses and addresses the physical, emotional, and social needs of patients, while working closely with healthcare providers and internal/external partners to remove barriers to care. ONNs provide education, improve access, and streamline communication to ensure a supportive, seamless experience for patients and their families.
Key Responsibilities Assess, plan, facilitate, and coordinate care to meet patients' physical, emotional, and social needs Advocate for timely, high-quality care by guiding patients through the healthcare system Coordinate appointments, referrals, and services to ensure continuous care Provide education on treatment plans, symptom management, and health maintenance Maintain accurate documentation and tracking of patient progress and clinical outcomes Reduce barriers to care through anticipatory guidance, insurance coordination, and community resource navigation Collaborate with oncologists, specialists, and support teams to enhance patient experience and outcomes Educate patients on individualized care plans to reduce unnecessary utilization and improve quality of life Serve as a trusted resource for patients and families throughout the cancer journey Perform RN scope duties, including telephone triage and clinical assessments Participate in clinic activities and assist in care coordination and planning Perform other duties as assigned Qualifications
Education & Experience
Graduate of an accredited nursing program (required) At least 1 year of nursing experience (required) Bachelor's degree in nursing (preferred) Outpatient nursing and scheduling experience (preferred) Credential/Licensure
Current RN license in the state of practice or compact license (required) BLS certification (required) OCN certification required within two (2) years of hire Skills & Abilities
Strong knowledge of oncology practices and patient navigation Excellent verbal and written communication skills Critical thinking and conflict resolution skills Ability to prioritize, manage time effectively, and work both independently and collaboratively Proficiency with electronic documentation and EMR systems Compassionate, patient-centered approach with strong attention to detail Why West Cancer Center is a Great Place to Work
