Description
Azura Palliative Care and Hospice of WI is seeking a compassionate and experienced Nurse Practitioner to join our dedicated hospice team. In this role, you will deliver advanced, holistic care to patients and families facing life-limiting illnesses. You will play a vital role in supporting patients and their families throughout the hospice journey, providing expert guidance and compassionate care every step of the way.
Key Responsibilities: Provide in-home and facility-based hospice care through assessment, diagnosis, and treatment. Conduct physical exams and face-to-face visits to determine hospice eligibility. Develop and manage individualized care plans in collaboration with the interdisciplinary team. Prescribe medications and treatments as appropriate. Educate and support patients and families on symptom management and disease progression. Coordinate care with physicians, nurses, social workers, and spiritual care providers. Maintain accurate clinical documentation in compliance with regulatory standards. Qualifications:
Current Registered Nurse license and certification as a Nurse Practitioner. Graduate-level education with training in hospice, palliative care, or geriatrics preferred. Strong communication, critical thinking, and clinical skills. Ability to work independently and collaboratively within a team environment. Valid driver's license and reliable transportation for patient visits.
