QUALIFICATIONS

:

Current and valid license by the Illinois Registered Professional Nurse from the State of Illinois Department of Professional Regulation Hold or be eligible for an Illinois Professional Educator License as a School Nurse Excellent verbal and written communication skills Bilingual skills helpful Strong organizational skills and ability to work with details and maintain accurate and thorough records Strong project management skills, including personal initiative and ability to coordinate with others

GENERAL DESCRIPTION:

Assessment

Prepares comprehensive health histories and pertinent medical reports. Participates in joint evaluation process with other disciplines. Prepares emergency health care plans for all students who need them and train classroom teams on the implementation of such plans. Assess environmental needs of students and make recommendations for adaptations. Arranges, supervise, and follow up on all physical assessment programs. Performs ongoing assessment of student progress and changes in students' needs based on ongoing systematic data collection and analysis.

Program Planning

Collaborates with other related service personnel to determine program needs. Plans for, administer and/or supervise the administration of any health-related procedures (i.e., medication distribution, gastrostomy tube care, etc.) necessary for a student to participate in the school program. Writes medical support plans for student with IEPs and/or 504s. Assists families and staff in securing available health & community resources. Arranges for sub nurse coverage in assigned buildings. Prepares a wide variety of written materials (e.g., activity logs, correspondence, memos,

treatment plans, Medicaid billings, reports, required documentation, etc.) for the purpose of documenting activities, providing written reference, and/or conveying information.

Program Implementation

Provides services to students in accordance with IEP requirements. Communicates to building personnel weekly schedule. Balances service delivery models between direct, indirect, and consultative. Secures any specialized equipment necessary to maintain the general health of

all students. Train staff on the use and maintenance of specialized equipment.

Secures required physical and dental examination and immunization records for all students. Keep all health records up to date. Documents progress, feedback, and recommendations for program changes. Maintains data, records of attendance, contacts and logs as required. Maintains confidentiality about pupils and families, both within and outside the school setting. Team Process and Professional Growth

Participates in staffing and annual review as required by IEPs. Acts as a liaison between school, student, parent, physical and/or community agencies regarding the health status of students in the program. Participates in and present in-service programs as necessary. Expands professional knowledge and expertise by attending staff development activities. Other duties as assigned by the Supervisor.

TERMS OF EMPLOYMENT

:

7.0 hours per day All student attendance days, Institute Days, and Workdays, with an additional five days as determined by the Principal. Salary to be established by the Board of Education. Commensurate with experience...starting salary for licensed staff is $52,932. Health Insurance and Dental Insurance Life Insurance Policy Sick and Personal Days

EVALUATION

: The Principal or designee will serve as the evaluator with performance reviewed annually in accordance with the provisions of the Board Policy on Evaluation of Personnel.

9 month position (181 day work year)
