Care Coordinators
supporting children and youth with behavioral or developmental health challenges - and their families.
As a Care Coordinator, you'll work closely with families and community partners to connect children and youth to the care and services they need to thrive.
What We're Looking For: High School Diploma with 3 years' experience
OR Associate/Bachelor's degree with 2 years' experience
OR Master's degree with 1 year of experience Background in behavioral health, child welfare, developmental disabilities, juvenile justice, or a related field Strong knowledge in family systems, case management, child development, or community resources Proficiency with computers/systems Valid driver's license, reliable transportation, and insurable under agency policy • LSW/LPC preferred
