Responsibilities The Oncology patient navigator has a unique role in patient care. As a patient advocate, this individual will build working relationships, solve problems, direct patients to resources both within the organization and externally in the community as well as support patients as they learn to self-navigate the healthcare system. The primary focus of the Patient Navigator is to support providers in multiple clinics coordinate the plan of care with the healthcare team. The RN Patient Navigator will assume a leadership role within the clinics for clinic personnel to prepare and assist them with their responsibilities.
Qualifications Minimum Qualifications:
At least 3 years in hospital or health care setting with accountability for working with patients in nursing and/or case management/care coordination
2-3 years experience in cancer care and treatment
Required training or certification in patient navigation or willing to successfully complete training in patient navigation
