RN Hospice Admissions Nurse
Job Description
Admission Nurse is responsible for identifying needs for terminally ill patients and their families/caregivers through the intake and admission process. Nurse will collect referral and insurance information as necessary and schedule admission as applicable. RN meets with patients/caregivers for informational visits and admission visits as assigned and works with physicians to establish initial plan of care and meet immediate needs.
Responsibilities Assesses and teaches effective and safe use of medical equipment and supplies as applicable., Assesses individuals for appropriateness for hospice care, Assessment- Collects pertinent data and information relative to the patient's health or the situation. Recognizes normal and abnormal findings when gathering data. , Assists department in performing quality monitoring audits and meeting urgent/unmet needs of patients/families with nursing visits when no admissions are scheduled., Attends to patients and families at the time of patient's death as applicable., Completes assessment of patient and family to ascertain and meet hospice needs including physical, emotional, spiritual and mental status., Coordination of Care- Plans and evaluates care in collaboration with appropriate disciplines for assigned patients., Diagnosis - Analyzes assessment data to determine actual or potential diagnosis, problems, and issues., Ensures quality and safe delivery of hospice services through the development and implementation of the patient's initial plan of care, in coordination with the Interdisciplinary Team and under the direction of the primary physician., Evaluation- Evaluates progress toward attainment of goals and outcomes. Reports data and outcomes to as appropriate to physician and/or other disciplines., Implementation - Implements care and supervises care delivered by other team members in alignment with the plan of care and approved safety and infection control standards., Knowledge - In collaboration with Leader/Director, actively pursues required knowledge and skills through orientation activities specific to the unit, unit based in-service programs, reading current literature and seeking new clinical learning opportunities. , May accept potential admissions from referral sources and coordinate admission needs with referral sources, families and scheduler., Outcomes Identification - Identifies expected outcomes for a plan individualized to the patient or situation., Participates in on-call rotation for after office hours, weekend, and holiday admission needs, Participates in scheduled meetings., Planning - Develops a plan that prescribes strategies to attain expected, measurable outcomes., Practices safe and effective universal precautions for infection control and teaches patient/family infection control protocols., Provides needed immediate nursing care and arranges for needed services, medications, supplies, and equipment for patients and families., Relationships - Establishes effective working relationships with peers, physicians, and other members of the health care team. Identifies and confers with appropriate resources regarding patient care decisions., Submits required documentation on a timely basis., Teaching - Employs teaching strategies to promote health and a safe environment., Uses specialized knowledge to effectively manage patient's symptoms. Teaches patient and caregivers the skills necessary to maintain patient's comfort and meet patient's goals., Utilizes and communicates with other team members, outside agencies, and community resources to ensure continuity of care., Verifies reimbursement sources (i.e. Medicare, Medicaid or insurance) prior to admission and provides information regarding reimbursement for services to patients/clients and families.
