$0.67 per mile (tracked via Kronos; from home health site to patient homes) Benefits Include: Paid Sick Leave Health Insurance Job Summary: Under the general supervision of the Nursing Supervisor, the Home Health RN provides skilled, compassionate nursing care to patients in their homes. This role involves conducting assessments, managing care plans, and collaborating with interdisciplinary team members to ensure high-quality, integrated home health care delivery. Key Responsibilities: Deliver direct, skilled nursing care in the patient’s home according to the plan of care. Conduct initial and ongoing assessments, including OASIS documentation. Administer IV therapy, wound care, and other nursing procedures as needed. Educate patients and family members on health conditions, medications, and home care procedures. Identify needs and coordinate with allied health professionals (PT, OT, Speech, Nutrition, Social Work). Update documentation in
Epic
and ensure timely submission. Maintain accurate mileage and time records for reimbursement. Adhere to safety and infection control protocols. Required Qualifications: Minimum
2 years of recent Home Health nursing experience Current
Michigan RN license Current
BLS certification
(through AHA) IV therapy experience Experience with
OASIS
and
Epic
documentation systems Valid
State of Michigan driver’s license Reliable transportation and a phone Clean driving record Ability to lift, move, and transport patients if necessary Preferred Qualifications: Acute care experience Strong interpersonal and communication skills Ability to work independently and manage a dynamic caseload About First Connect Health
