Your Responsibilities Will Include Performing thorough assessments of patients, ordering necessary diagnostic tests, and devising customized care strategies Diagnosing and managing acute, chronic, or episodic health conditions independently or with physician collaboration (as credentialing allows) Providing education to patients and families on wellness, prevention, and chronic disease management Prescribing appropriate medications and therapies within your scope of practice Engaging in patient follow-up and chronic care management initiatives Collaborating with multidisciplinary teams to offer comprehensive, cohesive patient care Maintaining precise documentation in electronic health records (EHR) Educational Background Required
Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) Graduate of an accredited Nurse Practitioner program Essential Certifications
Current Nurse Practitioner license in New York National certification as a Family Nurse Practitioner (FNP-C, AANP, or ANCC) or an equivalent specialty Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications (or the capability to obtain within 90 days) What Makes You Stand Out
