Job Responsibilities Conduct thorough evaluations for patients in both inpatient and outpatient environments, creating personalized treatment plans Deliver physical therapy interventions focusing on enhancing mobility, strength, balance, and functionality Work collaboratively with nurses, physicians, case managers, and other rehabilitation professionals to optimize patient outcomes Instruct patients and their families on therapeutic goals, discharge strategies, and at-home exercises Document assessments, treatments, and patient progress in line with facility and regulatory standards Engage in care team meetings and interdisciplinary discussions to ensure seamless patient care Education Requirements
Doctorate in Physical Therapy (DPT) or an equivalent qualification from an accredited program Required Certifications
State licensure as a
Physical Therapist CPR certification preferred Specialized certifications (e.g., neurological, geriatric) are advantageous but not mandatory Skills and Experience
Prior experience in physical therapy is highly desired Ability to provide care to patients with complex medical and mobility challenges Excellent communication and teamwork abilities Familiarity with digital charting and clinical documentation software Benefits Offered
