Description Conduct thorough evaluations of patients in both inpatient and outpatient environments and develop customized treatment plans Deliver physical therapy interventions with a focus on enhancing mobility, strength, balance, and overall functionality Collaborate closely with nurses, physicians, case managers, and other rehabilitation specialists to maximize patient outcomes Inform patients and their families about therapeutic objectives, discharge protocols, and home exercise routines Maintain accurate documentation of evaluations, treatments, and patient progress in compliance with facility and regulatory standards Participate in multidisciplinary care team meetings and case discussions to ensure seamless patient care Education
Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited educational institution Certifications
State licensure as a
Physical Therapist CPR certification preferred Possession of specialized certifications (e.g., neurological rehabilitation, geriatric care) is advantageous but not mandatory Skills
Prior experience is strongly preferred Proficient in treating patients with intricate medical and mobility challenges Excellent communication and teamwork abilities Familiarity with digital record-keeping and clinical documentation systems Benefits
