The application window is anticipated to close within 30 days of the date of the posting. Education and Experience Nursing Degree from an accredited college or university. Minimum two to four years related experience and/or training; or equivalent combination of education and experience. Long term care experience preferred. Certifications, Licenses, and other Special Requirements Must have valid and current state (RN) license, issued by appropriate state licensing agency. Physical Demands and Working Conditions Standing Walking Sitting Use hands and fingers to handle or feel Reach with hands and arms Stoop, kneel, crouch crawl Talk or hear Ability to lift up to 25 pounds Vision Requires interaction with co-workers, residents or vendors Responsibilities Assist the Director, Clinical Services in planning, organizing, developing and directing the day-to-day operations of the Clinical Services department in accordance with current federal, state and local standards. Assist in maintaining systems and established policy and procedures. See Infection Prevention Addendum for additional job duties. Assess the comprehensive nursing needs of each resident using approved long-term care assessment software tools and according to state and federal time frames. Coordinate care plans for each resident. Oversee day-to-day care to ensure care plans are followed. Work with team members to establish appropriate level of care in accordance with Medicare/Medicaid reimbursement levels. Assist the Director, Clinical Services in ensuring comprehensive and accurate documentation by nursing/direct care personnel. Monitor, manage and provide education on Clinical Systems, programs, and processes as assigned. Implement Clinical Systems and programs in accordance with current rules and guidelines governing long-term care. Monitor community Quality Indicators and survey reports; provide recommendations for Success Planning including in-service training on clinical skills and required topics. Plan, implement, evaluate, and direct the facility's Infection Prevention and Control Program. Apply scientific principles to surveillance and reporting of infection. Plan and conduct educational programs based on prevention, trends, and outcomes. Prepare reports and presentations for Quality Assurance Performance Improvement Committee, including Antibiotic Stewardship activities. Investigate and report outbreaks of infection and implement prevention interventions. Assist in policy/procedures to support compliance, infection prevention, and resident safety in adherence to OSHA regulations and infection control standards. Develop and implement Clinical Services organizational structure. Determine staffing needs; recruit, select, and orient nursing/direct care personnel. Lead staff development; develop work assignments and schedules; delegate authority to supervisory/lead staff. Review complaints and grievances; report and investigate allegations of abuse and/or misappropriation of resident property; report suspected violations of disclosure of resident protected health information. Assist with coordination of ancillary services. Collect, coordinate, and submit MDS data to the state. Monitor medication and treatment schedules; work with attending physicians; provide direct care when required. Maintain policies and procedures governing day-to-day operations of the Clinical Services Department. Ensure staff compliance with regulations; maintain safety standards for equipment and rooms. Monitor supply and equipment needs and budget. Monitor Medicare/Medicaid reimbursements. Complete other duties as assigned. This job description represents an overview of responsibilities.
