Position Summary
The Hospice Director of Quality Outcomes is responsible for modeling the Compassus values of Compassion, Integrity, Excellence, Teamwork, and Innovation and for promoting the Compassus philosophy, using the 6 Pillars of Success as the foundation. S/he is responsible for upholding the Code of Ethical Conduct and for promoting positive working relationships within the company, among all departments, and all external stakeholders. The Hospice Director of Quality Outcomes assesses the quality of care provided to hospice patients, families, and staff. S/he monitors documentation, compiles regular reports, and works closely with the program leadership. Position Specific Responsibilities
Owns ensure H.A.R.S (Hospice Aide Supervision, Assess skin/wound status, Reconcile Medications, and Screen for Pain (and intervene for comfort)) audits are completed and accurate. Leads/Ensures outside audits are responded to accurately and timely. Ensures support with the Areas Executive Clinical Operator PreBill is completed accurately and timely. Manages clinical action planning for services scores and Star Ratings. Manages We Honor Vets with a focus to move each location 1 level annually. Ensures internal scorecard follow-up and POC effectiveness. Ensures QAPI is accurate and complete. Participates in the development and update of patient care policies and emergency procedures. Collaborates effectively with Regional Director of Clinical Operations, Area Director of Clinical Operations, Director(s) of Clinical Services and members of senior leadership team to assure clinical excellence in each location. Performs documentation reviews and audits to assess:
Quality of clinical documentation across all members of the IDT. Evidence of care coordination. Evidence of plan of care revision according to assessments. Compliance requirements are met. Documentation of Mock surveys in locations to assure compliance of hospice regulations.
Reports deficiencies to appropriate staff for corrections and follows up to ensure completion. Assists program leadership with development and implementation of plan of correction following internal/ external survey/ audits, updates supervisor when a plan of correction validation intentional site visit is to be completed. Audits IDT documentation for accuracy and completeness. Supports Area Director of Clinical Operations to ensure continuous CHAP readiness. Compiles quality data on a quarterly basis and submits summary to Hospice Area Director of Clinical Operations for Governing Body reporting. Supports clinical onboarding Assures annual competency and training for MD and Teammates is completed and filed accurately. Participates in performance improvement programs and assures oversight of locations Performance Improvement plans. Upholds Compassus philosophy of integrity and commitment; communicates proactively when schedule changes are needed. Models and facilitates communication between teammates including medical records, business office, intake, clinical team, and leadership. Provides input into program service recovery efforts. Solicits and incorporates internal team member feedback into quality efforts and performance improvement. Performs other duties as assigned. Education and/or Experience
Bachelor's in Nursing strongly preferred but other degrees will be given consideration. Three (3) years of clinical experience required. Experience in quality assessment, quality management, or data analytics a plus. Experience with electronic medical records systems a plus. Resides in State of oversight preferred. Ready to Join?
